The MODA delta dental PPO plan offers in- and out-of-network benefits, providing you the freedom to choose any provider. However, you will pay less out of your pocket when you choose an in-network PPO provider. Expenses from Premier and out-of-network providers are reimbursed based on reasonable and customary (R&C) charges. Any charges over the R&C amount will be your responsibility.
